We're delighted to have your family enroll in Christian Pilgrims Schools, Internation. This checklist is provided to help you make certain that all necessary enrollment forms are filled out correctly and completely. If questions remain after reviewing the directions feel free to contact us for clarification. Before contacting us make certain that you have all necessary forms. You will likely need to make copies of certain forms if you are enrolling more than one student.
All Families
Annual Family Survey — CPSI deals as much as possible, with whole families. Even family members not officially enrolled as students are part of the God-ordained training environment and should be reflected on the survey. Answer each question honestly and fully. Answers on the survey are not used to determine admission into the school, but rather to gauge the discipleship state of our school community and to aid you in honestly facing issues important to godly families. At the conclusion of the survey you are asked to write a statement of your family's educational philosophy. Both parents, and possibly the older children, should participate in creating or revising this statement each year. We suggest that you have it neatly typed and make copies available to inquiring friends. Re-enrolling families are asked to review and revise these statements annually. Families new to CPSI are also required to provide individual written statements from each parent as to the state of there personal relationship with Jesus Christ. This is to be completed by each parent regardless of what that relationship is.
Commitment Sheet — To avoid possible future misunderstandings, each family is asked to review, complete, and sign a Commitment Sheet spelling out CPSI's expectations. Be certain that you understand and are comfortable fulfilling all seven commitments. If you have any questions or reservations please discuss them with us. On commitment #7 you are asked to give us an idea of what you expect to be able the contribute to the ministry monthly. You will not be held accountable to the monthly amount you list on the form. This is simply to give us an idea of the amounts we might plan on for budgeting purposes.
Independent Study Learning Plan (one per student) — Select the appropriate form elememtary or secondary) for each student and write a proposed plan that you wish to be held accountable for implementing. These written plans may be revised at any time you (or we) feel they are no longer realistic or desirable in their current form. It is recommended that the plan be revised once each semester, especially for older children. The plan must not be writtenfor more than the current school year. Once your proposed Learning Plan is accepted it becomes, allong with your Signed Commitment Sheet, the basis for your accountability to CPSI. Learning Plans for high School students should include course titles to be entered on the transcripts, along with a description of the content, resources, methods, and evaluation strategies for each course. Those High Schoolers who will never need transcripts may use Elementary Learning Plan forms (First time home educators often find the task of devising proposed Learning Plans Nearly Overwhelming. To simplify the process initially, just study the examples in your Enrollment Pachet and Listen the Descriptions in the Orientation Meeting and adapt the ideas presented there until you have better ideas of your own. We can provide advice f it is desired. You will soon become an expert at your children's edeucation yourself.)
Educational Philosophy — We ask each family to write out (and then annually review and revise) a statement of what they believe about education. We encourage you to thoughtfully ponder God's design for the training of children, and the preparation for their future. This need not be complicated or sophisticated. Simply describe what you currently understand about education in terms of what, why, who, when, where, and how.
(Father's and Mother's) Personal Relationship with Jesus — Upon your family's initial enrollment, we ask each parent to write out a statement of their current relationship with Jesus. You might share the story of how you came to know Him, or you could describe the essence of that relationship. If you don't currently have much of a relationship with Him, simply tell us that. This can be short or long, depending on what you want to share.
New Students (forms)
Individual Student Enrollment Application — Please fill these out completely. All items are self-explanatory. It is particularly critical that we have the complete address of the most recent school of enrollment in case there is a question about cumulative records (a phone number may be helpful as well if you have it).
Request for Student Records — Please Please fill out and return this form to us. If you wish to send this request directly to the previous school please still send us a completed copy of the request form in case we need to repeat the request. This form also serves to notify the previous school that your children are no longer enrolled there. You do not need to contact the school yourself or inform them of this unless, for some reason, you wish to.
Immunization Records and Health Examination for School Entry Report or Waiver — These legally required enrollment documents are only necessary if they are unavailable with the records of your student's previous school.
Monthly Forms (forms)
Fathers Monthly Plan Sheet or Daily Activity Logs — The Faather's Monthly Plan Sheet looks very similar to the "Independent Study Learning Plan" (that form can be used, if this one is unavailable for some reason), but is to be used monthly to aid in fathers planning with their wife. If the father of the family is unable or unwilling to lead the planning and accountability process, the mother should instead use the Daily Activity Logs. Then either the Father's Monthly Pan Sheet or the Dalily Activing Logs are to be used in your monthly accountability meeting for reporting on your planning & progress, and submitted to CPSI.
Meeting & Accountability Report Form — This form serves as a cover sheet for families in the CPSI Extension Program, as you send in your monthly documentation (work samples and either Father's Monthly Plan Sheet or Daily Activity Logs.
